Thank you for considering care with our office. We have had excellent success in caring for thousands of clients, with hundreds of different conditions, in the 16+ years we have served the East Valley. However, we do not accept all cases, as we are not able to help everyone.
Our focus is on wellness care for our clients, with the goal of
restoring your body’s ability to heal itself. We address not only your symptoms, but improving the lifestyle and
habits that created and perpetuated your concerns.
Please start this process by reviewing our policies to see if we are a good fit for one another.
How to begin your care:
Please fill out and submit our "Application for Care", prior to your consultation with our office. Your application must be received at least 24 hours prior to any scheduled consultation time, so as to give us time to review your situation, and help us determine if we can be of service to you.
To schedule your consultation, you have several choices. You may call our office, or go online (see the link at the bottom of the page) to schedule with Dr. Jeff. Or, if you would prefer, you may wait until after we receive and review your application, at which point we will call you to schedule the consultation. The consultation fee is $50, and will take approximately 30-45 minutes.
If after receiving and reviewing your application we believe we can't be of service to you, we will call you to discuss other options which might better serve your needs, prior to the consultation.
Consultation:
At this time, Dr. Jeff will review your application with you and ask some detailed questions regarding your concerns, as well as your lifestyle. At the conclusion of the consultation, If it feels like a good fit for both you and Dr. Jeff to work together, we will then schedule the examination of your three dimensions. The examination fee is $270 and must be paid at the time you schedule.
Examination:
Our work focuses on balancing the three dimensions of life: the physical (muscles, joints and movement), the biochemical (nutrition and diet) and the emotional/energetic dimension (energy or "chi"). Our examination looks at each of these three dimensions, and identifies any areas of imbalance. These imbalances are the cause of stress to your body and nervous system, and limit or stop the body from fully expressing its natural health, balance and energy. At the conclusion of the examination, we then schedule time for your report of findings, during which we will present the results of your examination. The fee for this is included with your examination fee.
Fees for our care:
After we present your report of findings, we will make our recommendations for your care, although it is ultimately your choice on how you would like to proceed, based on your needs and any time or budgetary constraints. Our fees are priced per case, which includes not only the type of care you receive, but the time and intensity involved.
Payment policies:
Payment is expected at time of service, unless other arrangements are made prior to your visit. We accept cash, checks, and Mastercard, Visa, and American Express. We have a number of payment plans available to you to help make your care more affordable.
Insurance Coverage
We do not accept any insurances within our office.
We understand that insurance coverage may help lessen any financial concerns. However, you are our first and only concern, and we answer only to you regarding your health and wellness choices. Our services are private, discreet, and any decisions about your care are made by you and Dr. Donahue directly.
Medicare
We do not accept Medicare, but we will be happy to refer you to another office that does. Our care does not typically qualify for medicare reimbursement.
Personal Injury or Auto Accident Claims
If your care is qualified under a personal injury or auto accident claim, we will bill these services for you.
Your individual case will need to be reviewed to determine if it qualifies for billing. If we do bill for these services as a courtesy to you, you will be charged our normal, full fees for services. Additionally, you are responsible for all charges in your care, including any charges not reimbursed by the insurance company.
Workers Compensation Injury Claims
We do not accept or bill for workers compensation injury claims in this clinic.
Appointments
Most appointments, with the exception of the consultation, examination, and report of findings, usually last from 10 minutes to 30 minutes.
If you are late for an appointment, your care time may be reduced in order to finish on time for the next scheduled client.
Your appointment is a time slot reserved for you, and we may charge the minimum appointment fee for missed appointments unless a 4 hour notice is given.
If you would like additional time to consult with the doctor, please call prior to your visit. This will allow more effective scheduling of your time.
